Booking and Collection of Information
Appointment booking and the collection of necessary personal information is handled by IntakeQ, Inc. For their terms of use and privacy policies see the following links.

Credit Card Billing
Credit card billing is processed by Square, Inc. For terms of service, privacy policies, payment terms, and other legal information, see the following link.

Payment
Appointments booked online through IntakeQ require a credit card for confirmation. The credit card used to confirm the appointment will be charged 6 hours before the appointment. Cancellations after that point may incur a 25% cancellation fee.

Cash, valid check with the date of service, and in person credit card payments are also accepted. Payment is due in full when services are rendered or goods are purchased.

Per Maryland law, bounced/returned checks can garner a bounced check fee plus an amount up to two times the value of the original bounced check, not to exceed $1000. Frederick Body Work, LLC will charge the standard Maryland fee plus the bad check return fee charged by our bank. Upon occurrence, the client will be billed for these fees in addition to the original check amount. Failure to remit appropriate remuneration (cash, certified check or money order) within 10 days will result in a filing of an Application for Charges with the Maryland courts. Frederick Body Work, LLC reserves the right to charge a higher bounced check fee as well as refuse future services if a client repeatedly issues bad checks.
Cancellations and Rescheduling
If you need to cancel or reschedule an appointment, you can do so by clicking on the “Manage Appointments” link on the upper right hand side of the main booking page.

You may also cancel or reschedule by email, text, or phone. Please plan to cancel or reschedule as soon as you are able. Cancellations 6 hours before the appointment time may incur a 25% cancellation fee.
Lateness / No-Show
Appointments begin and end at the scheduled time.

If you book an hour at 3pm, you get our undivided attention until 4pm. Then we’ve got 15 minutes to transition over to our next client at 4:15pm. And so on and such forth.

When you’re late, you miss out on some of the time you paid for. That’s all.

If it is ever necessary for us to be late or absent, we will let you know as soon as possible, offer you the option of rescheduling, and reimburse you the value of the missed time.

If you fail to show for a scheduled appointment and give no notice, you may be billed for the appointment in full.
Privacy and HIPAA
All client notes and records in possession of Frederick Body Work, LLC are confidential and will not be disclosed except where required by law. Information will only be shared with other health care providers for consultation or coordination of care with your consent, or, as warranted, with agencies involved in payment / reimbursement for services provided to client. Information may also be used for the sending of emails and texts pertaining to scheduled appointments and informational forms.

Though we make every effort to preserve user privacy, we may disclose personal information when required by law or under the good-faith belief that such action is necessary under applicable law. We may disclose personal information in order to establish or exercise our legal rights or defend against legal claims. We may also share information in order to investigate, prevent, or take action regarding illegal activities, suspected fraud, to protect our property or interests or that of our agents and employees, and to protect personal safety or the public.

We do not retain, share, store or use personally identifiable information for any secondary purposes. If you have subscribed to our blog/newsletter, participated in a promotional event or provided us with your contact information by any other means, please rest assured that your personally identifying information will never be sold or disclosed to any parties outside of Frederick Body Work, LLC, except as described above or where required by law.
IntakeQ, Inc, our booking, messaging, and intake form processing client, is HIPAA compliant.

Persons Under the Age of 18
Persons under the age of 18 are required to have a parent or guardian sign the new client intake form before they can receive treatment. The parent or guardian must be present in the therapy room for each appointment.
Affiliate Program Disclosure
We are a participant in the Amazon Services LLC Associates Program, an affiliate advertising program designed to provide a means for us to earn fees by linking to Amazon.com and affiliated sites.
Business Transitions
In the event that Frederick Body Work, LLC goes through a business transition, such as a merger, an acquisition by another company, or a sale of all or a portion of its assets, client’s personal information will, in most instances, be part of the assets transferred. Users will be notified by email and via prominent notice on our website for 30 days after a change of ownership or control of their personal information, if personal information will be used contrary to this policy.
If as a result of the business transition, the users’ personally identifiable information will be used in a manner different from that stated at the time of collection, users will be given the option to opt out consistent with our notification of changes section.

Nothing in this Policy is intended to interfere with the ability of Frederick Body Work, LLC to transfer all or part of its business and/or assets, for any purposes, without any limitation whatsoever. Frederick Body Work, LLC specifically reserves the right to transfer or share a copy of personally identifiable information to the buyer of that portion of its business relating to that information.
